Funding availability for the HUD Indian Community Development Block Grant program,
was announced in the Federal Register, Vol.70 No. 53, March 21, 2005, Notices.
Both the General Requirements and Program
Requirements were included in that announcement. The following was excerpted
from those announcements.
"Application Screening -The Area ONAP will
screen applications for single purpose grants. The Area ONAP will reject an
application that fails this screening and will return the application unrated.
The Area ONAP will accept your application if it meets all of the criteria listed
below as items a through f: a. Your application is received or submitted in
accordance with the requirements set forth under Application and Submission
Procedures in Section IV of this Program NOFA; b. You are eligible; c. The proposed
project is eligible; d. Your application contains substantially all the components
specified in Section IV.B. of this Program NOFA; e. Your application shows that
at least 70 percent of the grant funds are to be used for activities that benefit
low-and moderate-income persons, in accordance with the requirements of 24 CFR
1003.208. For screening purposes only, HUD will use the 2000 census data if
the data you submitted does not meet this screening requirement; and f. Your
application is for an amount that does not exceed the grant ceilings listed
in Section IV.E.2."
"Application Information - To expedite the review of your application
and ensure that your application is given a thorough and complete review of
all responses to each of the components of the selection criteria, please indicate
on the first page of each project submission, the type of project(s) being proposed:
Economic Development, Homeownership Assistance, Housing Rehabilitation, Land
Acquisition to Support New Housing, Microenterprise Programs, New Housing Construction
or Public Facilities and Improvements. This will help to ensure that the appropriate
project specific thresholds and rating subfactors will be applied. Narrative
statements should be individually labeled to reflect the item the narrative
is responding to, e.g. Factor 1, Capacity; Factor 2, Need; etc."
"It is recommended that you limit your narrative explanations to 200
words or less and provide necessary data such as a market analysis, a pro forma,
housing survey data, etc., that support the response. Include all relevant material
to a response under the same tab. Only include documentation that will clearly
and concisely support you response to the rating criteria."

Appendix B: Forms. The following forms
are required for your ICDBG application.> 2005 ICDBG APPLICATION CHECKLIST
1. _____ Application for Federal Assistance (SF-424).
2. _____ Applicant/Recipient Disclosure/Update Report (HUD-2880).
3. _____ Acknowledgment of Application Receipt (FIUD-2993).
4. _____ Client Comments and Suggestions (HUD-2994), optional.
5. _____ If applicable, concurring resolutions from the Tribe(s) if the
application
is prepared by a "Tribal Organization" for one or
more eligible applicants.
6. _____ Community Development Statement that includes:
6a. _____ Components addressing relevant threshold requirements and rating
factors.
6b _____ Cost information by project, including specific activity costs,
administration, planning, and technical assistance (if any),other dollars
to be included in the project, and total HUD share. (Use of HUD-4123,
Cost Summary, is required)
6c _____ A schedule for implementing the project. (Use ofHUD-4125, implementation
Schedule, is required)
7. _____ If applicable, a map showing project location.
8. _____ If applicable, displacement or temporary relocation information.
9. _____ If applicable, evidence of public disclosure (24 CFR 1003.606,
Conflict of Interest).
10. _____ If applicable, survey and demographic data statement (see sample
in Appendix B).
11. _____ Logic Model, HUD 96010 (optional for use with Factor I and Factor
5).

The following was excerpted from the Federal
Register, Vol. 70 No. 22, February 3, 2005, Notices:
"A complete application for assistance under
this Program Announcement consists of three parts. Part One includes the SF 424,
other required governmental forms, and other required documentation. Part Two
of the application is the project narrative. This section of the application
may not exceed 40 pages. The line item budgets, budget justifications and the
OWP form (OMB Control Number 0980-0204, exp 10/31/2006) will be exempt from
the page limitation. Part Three of the application may not exceed 20 pages (the
exception to this 20 page limit applies only to projects that require, if relevant
to the project, a Business Plan or any Third-Party Agreements)."
"ANA Application Format: ANA requires all application
to be labeled in compliance with the format provided in the program announcement.
This format applies to all applicants submitting applications for funding. All
pages submitted (including Government Forms, certifications and assurances)
must be numbered consecutively (for example, the first page of the application
is the SF 424 and must be labeled as page one). The paper size shall be 8.5
X 11 inches, line spacing shall be a space and a half (1.5 line spacing), printed
only on one side, and have a half-inch margin on all sides of the paper. (Note:
the 1.5 line spacing does not apply to the Project Abstract Form, Appendices,
the Table of Contents, the Objective Work Plans, and the Budget.) The font size
shall be 12-point and the font type shall be Times New Roman."
